Physical and Emotional Healthcare

Helping your family thrive.

Room to run. Opportunities to learn good, old-fashioned life skills. Safe, close-knit neighborhoods. There are a lot of wonderful things about raising kids in small rural communities like ours. But when you’re already doing the toughest job in the world, getting them the right care when providers are an hour away can be a huge obstacle to overcome.

Our Community Resource Centers are here to help. Located in Mapleton Middle and Siuslaw Elementary Schools, we make getting your kids the care they need as simple as walking down the hall. No days off work. No long drives. No months-long waiting lists.

Behavioral assessment and early intervention.

Child, teen and family counseling.

Wraparound and referral services.

Open to all student families - no insurance requirements or income minimums/maximums.

Convenient, School-Based Locations

Our centers are located on the school grounds of Mapleton Middle and Siuslaw Elementary Schools, providing a safe, convenient location for your family to access care. And because our centers are run by PeaceHealth, you can trust that our staff and providers are knowledgeable and compassionate.

Our locations will begin by offering counseling and telehealth services as well as wraparound care and referral assistance. Created with flexibility in mind, we plan to expand our services to include additional medical care and educational opportunities in the future. For Patients

The services of the Mapleton and Siuslaw Community Resource Centers will be available to all students and their families beginning in September 2019 – and we expect to add more services in 2020! About Us

The Mapleton and Siuslaw Community Resource Centers are a collaborative effort brought to you by the Western Lane Behavioral Health Network. A grassroots coalition of local leaders in healthcare, education, social services and business, seek to create collaborative, adaptable, sustainable programs to help individuals and families on the central Oregon coast access the preventive and emotional healthcare they need to thrive.

Our resource centers are contractually run by PeaceHealth. Contracting with a trusted existing healthcare system allows us to provide the very best care and customer service as well as convenient services like online access to your records and bill pay that would otherwise be too costly for the network to offer.
